 We
want your hospital stay to be as comfortable as possible,
and we believe it should begin with a smooth and expedient
patient registration process.
All patients should remember to bring their Social Security
card, all active insurance cards, picture ID, complete list
of their medications and power of attorney (if applicable).
Patients are encouraged to leave their valuables at home to
ensure safe-keeping.
Inpatient Registration Information
To pre-register for all procedures, please go to Woodland
Heights' Diagnostic Center, which is located at 125 Gaslight
Medical Parkway.
Patients may pre-register between 7:30 a.m. and 4:30 p.m.
Monday through Friday. If a patient needs to check into the
hospital after hours or during the weekend, they are advised
to go the Emergency Room.
Those admitted for inpatient procedures should remember to
bring their robe, slippers and toiletries.
Outpatient Registration Information
When checking in for inpatient or outpatient services, you
may be asked to provide personal and financial information
about yourself. You also will be asked to sign a consent for
treatment and receive a copy of Patient Rights and
Responsibilities.
The information you share with our trained admissions
representative becomes part of your medical record and is
kept confidential.
Some patients are required to undergo simple pre-admission
testing prior to admission. These tests are typically
scheduled by your physician's office up to five days prior
to surgery. For your convenience, Pre-Admission Testing
(PAT) or registration is also available at the Diagnostic
Center. Hospital representatives will make sure test results
are communicated to your physician prior to your admission.

Outpatient Diagnostic Procedures - 30 minutes prior to
appointment time.
Outpatient Surgery - For your convenience, appointments can
be scheduled. Surgical pre-admits are seen on a "first come
first served" basis. This process may take up to 1 1/2 hours
depending upon work orders.
